How to deactivate your account
You can delete your account directly in the app.
Steps
This deletes your account and associated data from the app.

Log in
Open Provider App and sign in to your account.

Go to Security
Navigate to Settings → Security.

Delete Account
Tap Delete Account and follow the prompts.
